In general, a physician you choose will take care of you during the pregnancy
and will deliver your baby. You will be able to develop a close relationship
with your physician. If your physician is not available after hours then her
partner on-call will take care of you.
Gynecological Care
As gynecologists trained in the latest medical and surgical approaches we will
serve
you with the best practice of female reproduction and health care. In our
practice we emphasize preventive medicine. Our services provide:
- Annual gynecologic exams and care
- Preventive health care, including mammograms and Pap smears
- Screening and early treatment of gynecologic cancers
- Gynecologic surgery, including major pelvic surgery and laser surgery
- Hysterectomy and alternative treatments
- Advanced laparoscopy and hysteroscopy
- Colposcopy and cryosurgery
- Endometriosis and fibroids treatment
- Contraception counseling and prescriptions
- Personalized menopause management, including counseling, and hormone replacement options

- Pre-pregnancy counseling
- Ultrasonography
Your scheduled office visit will be with your primary physician. In emergency
situations when you must see your physician and she is not in the office, her
partner will take care of
you.
Emergency Care
If you have an urgent medical problem call our office at (408) 288-9933. We
strongly encourage you to call us during regular hours when you feel
that you have medical problem. This allows us to arrange for your care on a non-emergency
basis whenever possible. We may not always be available due to our profession
but we will have another physician contact you should
this be necessary.
After-hours our answering service will
help you in contacting our physician on-call that day. Please use this
after-hours service only in a medical emergency. If you
believe that you are in labor, call us before going to the hospital. This may prevent unnecessary emergency room visit and
the associated fees. However, if
this is a life-threatening situation, do not hesitate to call 911 or go to the nearest hospital. We will be contacted by the hospital staff.

To Make, Cancel or Reschedule an Appointment
To make, cancel or reschedule an appointment, please call (408) 288-9933 during office
hours. If you are making an appointment, please inform us of the reason of your visit so we can allocate the
proper amount of time for your visit. Our office visits are by appointment only. If you have an acute problem, we will try to accommodate you the same day.
At
the time of your appointment, please inform us of any changes in your insurance
coverage.
We respect your time and we will make every effort to honor your appointment on time.
Unfortunately our specialty involves occasionally unforeseen emergencies that may alter your physician's schedule.
We ask you for your understanding if that happens. We will make our best
effort to inform you ahead of time to minimize any inconvenience that may cause.
If you must cancel or reschedule your appointment, please give us at least 24-hour
notice. There is a charge for a missed appointment.
If This Is Your First Visit
If this is your first visit, please arrive 15 minutes early to fill out the medical
history and registration forms, and allow us complete your records.
Have your medical insurance card and any of your important medical records with
you. Please inform us about any health problems, allergies and any medications you are taking.
If
You Have Medical Problems or Questions
We strongly encourage you to call our office during regular office hours if
you have any health problems, need a prescription, have questions or concerns, or
you need to get touch with us for any other reason. Our front office staff will
either direct your call to the appropriate person, or take a message if this is
a non-urgent call or to schedule an appointment.
If your call requires
medical assistance and our physicians or medical assistants are busy with other
patients, the front desk will take a message and give it immediately to our medical
staff. If your call is urgent our physicians or medical assistants will call you as soon as possible. In non-urgent
cases they will make the best effort to call you back most likely later in the
day.
If your call relates to prescription needs, please make sure to
call during our office hours when your medical records are available to us so we
could provide the most appropriate treatment for you. Please have the pharmacy
phone number available to allow us to order your prescription.
If you are calling
regarding lab results, please allow our front desk to look up your results. In
general it is not necessary to call us about your lab results because we will
notify you as soon as possible it they are abnormal or require an early follow-up.
Usually your lab results will be discussed with you during your next visit.
Please advise us what phone numbers or address we should use to report abnormal
lab results. Make sure that our records reflect your correct address and phone
numbers in case we have to notify you.

Insurance Claims and Payments
Forest OB/GYN Medical Group works with most insurance plans. Our billing
manager will assist you in facilitating the reimbursement process by submitting
insurance claims.
Payments for Co-Pays, Deductibles and Non-Covered
Services are due at the time of your appointment. If you do not have insurance, your service fee
is payable at the time of your visit. We accept checks or cash only.
Please remember that your insurance contract is between
you and your insurance company and not our office. You are responsible for the full
payment of your account when due. Balances remaining after insurance reimbursement
will be due within 90 days from the day of service.
